Plana Care Plans for Unexpected Pregnancies

At Plana, we understand that an unexpected pregnancy is overwhelming, intimidating, and difficult to manage. That’s why our dedicated team crafts pregnancy care plans for each individual according to their needs.

Specifically, our app and care coordinator assesses the needs of each of our clients, while a dedicated mentor is there to help each individual on their journey. 

How is My Care Plan Built?

Once downloading the Plana app, you are prompted and asked questions to understand your individual baseline needs and priorities. Afterwards, the plan is then personalized through careful interaction and conversational questions with a dedicated care coordinator. Afterwards, the individual’s needs are noted, while resources are allocated accordingly.

What is Included in a Plana Care Plan?

As each pregnancy journey is unique and varied, so too are our care plans. Therefore, depending upon the needs of the individual, care plans may include, but are not limited to:

  • Medical support and resources
  • Adoption support and resources
  • Counseling 
  • Housing and transportation assistance
  • Job skills development
  • Childcare options and resources

After Giving Birth, Does My Care Plan Continue?

Plana will support women up to birth and individual circumstances will determine when resource-intensive support shifts, usually 2 months after birth.
